If a species is known to suffer from ISD, and the abdomen swells, it is a reasonable thing to believe that the problem is ISD. But very few species are known to suffer from ISD.
One very common disease also produces the same effect - atoxaplasmosis. This disease is very common, but hard to diagnose and very rarely diagnosed - it also produces a swollen abdomen by the same way as ISD - it can make the liver very large. Also megabacteria can produce the same symptom - also difficult to diagnose but also very common.
Unless a laboratory has analysed the liver of a bird and found ISD, you cannot say that any bird has died from ISD. A swollen abdomen is many, many times more likely to be caused by something that is not ISD.
Also - do not forget - many individual birds were kept for many years, by many people, using universal foods with many 100's or 1000's ppm iron. ISD is a very "new" disease, but it affects very few birds.
